Key Terms Glossary – Support at Home

Participant
A person receiving services under the Support at Home program. This term replaces older labels like “client”, “consumer”, or “care recipient” to better reflect choice, control and active involvement.

Support at Home
The new in-home aged care program, starting from 1 November 2025, which replaces Home Care Packages (HCP) and Short-Term Restorative Care (STRC). It focuses on helping older Australians live independently, safely, and with more personalised support.

Care Plan
A document developed in partnership with you, outlining your goals, preferences, and the types of support you’ll receive. It’s flexible and reviewed regularly.

Assessment
A process used to determine what services and level of support you may need. This is usually done by an Aged Care Assessment Team (ACAT) or the new Regional Assessment Service under Support at Home.

Package / Support Plan Level
Support at Home will offer different levels of care depending on your needs. The Government will fund the level that matches your assessed support needs.

Income-Tested Fee
A contribution some participants may be asked to make, depending on income. It’s assessed by Services Australia, not Baldwin Living HomeServe.

Approved Provider
An organisation (like Baldwin Living HomeServe) that meets strict Government standards to deliver aged care services under Support at Home.

Charter of Aged Care Rights
A document that outlines your rights as a participant. These include respect, privacy, independence, choice, and the right to give feedback or make a complaint.

Care Partner
A person from Baldwin Living HomeServe who helps coordinate your services, ensures your care plan is working, and supports your ongoing needs.

Service Provider
An individual or organisation that delivers a specific type of support — e.g., personal care, gardening, nursing, transport. They work under agreement with Baldwin Living HomeServe to support you.
