Our Care Partners are experienced in supporting individuals and families through every stage of the dementia journey. We are here to assist both the person living with dementia and their carers—because we understand that the health and wellbeing of carers is just as important as the person they support.

With the right supports in place, people can continue to live safely and confidently at home with dementia. We recognise that caring for a loved one with dementia is both a complex and evolving challenge, as well as an extraordinary act of love. That’s why we offer practical support, including respite services, to give carers time to rest, run errands, or maintain social connections. We also provide services that enable people living with dementia to continue enjoying their interests and hobbies in a safe and supportive environment.

We are committed to walking this journey alongside you. A wide range of resources, strategies, and services are available—and often, the right support can make a real difference in the lives of both the person living with dementia and their familie.

Our Care Partners can:

  • Deliver services and supports tailored to your individual needs—flexible and regularly reviewed
  • Refer you to specialist teams, with your consent, when additional expertise is needed
  • Provide practical resources and guidance when you’re unsure where to turn
  • Collaborate with you to develop dementia-specific care plans
  • Organise and fund relevant assessments
  • Assist with applications for subsidies and appropriate support programs

Carer Gateway

Government-funded support for unpaid carers – practical, financial, and emotional.

Carer Gateway offers free counselling, emergency respite, peer support, and coaching to people caring for a loved one.
How HomeServe helps: If you’re a carer for one of our participants, we’ll connect you with Carer Gateway, complete referrals on your behalf, and advocate for the help you need.

Older Persons Advocacy Network (OPAN)

OPAN supports older Australians to understand their rights, resolve care issues, and raise concerns confidentially and without fear.
